About Us
Adult Day Health Care Programs provide
services to thousands of frail, elderly, disabled and chronically
ill in California each year. ADHCC are making it possible
for them to continue living in the community while maintaining
a degree of independence. Our Centers have developed many
specialized programs, such as a Deaf and Hard of Hearing program,
an Alzheimers and Dementia program, a Multiple Sclerosis program,
along with many others.
Adult Day Health Care provides a broad spectrum of services
in a congregate day setting, including medical care, case
management, speech therapy, rehabilitation, meals, personal
care, recreation, education, and round-trip transportation.
Individuals enrolled in an adult day care program are able
to live at home on their own or with some assistance. The
Adult Day Health Care program will arrange for a participant
to be picked up at home in the morning, brought to the center
to participate in our program, and returned home safely.
Adult Day Health Care provides a wide variety of programs
and services to meet the ethnic needs of each participant.
Some of these services include multi-lingual staff, ethnic
meals, special events for the holidays and culturally sensitive
programs.
Most programs serve participants with a range of functional
or cognitive impairments, including Alzheimer's disease and
other dementias, chronic illnesses such as cardiac disease
and diabetes, traumatic brain injuries, developmental disabilities,
and other types of increased care needs. While some seniors
with these conditions may need the 24-hour highly skilled
nursing care provided by a nursing home, many are able to
manage on their own with help from family members and the
services offered by HZOR Adult Day Health Care programs.
Adult Day Health Care promotes good health. Individuals must
be referred by their physician, but
through the implementation of an individualized care plan,
should experience fewer office and emergency room visits.
This is mainly because they are monitored and have their health
needs met early on. Programs are staffed with sign language
interpreters, nurses, dieticians, and physical therapists
that provide assistance, regular monitoring, and tests for
illness such as diabetes and hypertension. HZOR ADHCC takes
a proactive approach in our participant’s health care
needs.
In addition to the services we provide in our program, our
social workers are on staff to assist participants in applying
for services such as home care, Meals on Wheels, public assistance,
and other benefits available to seniors.
Because social activity plays such a large role in maintaining
good health, recreational therapists or activity coordinators
are in charge of classes in various areas such as in language,
music, arts, exercise, and other leisure activities that help
seniors stay connected to the world around them. Through cognitive
games and peer support our participants show decreased social
isolation (a growing concern in the senior Community).
From the family's perspective, Adult Day Care is an attractive
option that lets them care for their aged parents at home
without giving up the employment income they need to finance
their children's education and provide for their own retirements.
Adult Day Health Care services may be funded through private
insurance or families may pay on their own, but most
often participants become eligible for these services through
the Medi-Cal program.
